In the second phase, we will define your future by reflecting on your past, clarifying your aspirations, and making informed decisions about your goals and priorities

Connecting the Plan
In the third phase, we will build your support network, identify useful tools, and start setting concrete goals to turn your vision into actionable steps.

Working Towards the Future
In the final phase, we will solidify your plan, address potential challenges, and celebrate progress by reviewing and refining your path forward.
Meet the Planners

Austin Scott
Austin is one of our dedicated planning facilitators, bringing over four years of experience in service of people with disabilities and a personal connection to the I/DD community. Growing up with a family member with a developmental disability, he understands the importance of person-centered planning and the impact it can have.
Fun fact: Austin is a triplet! Growing up in a large family, he saw firsthand how everyone has unique needs and goals—core to person-centered planning.

Andrea Gonzalez
Andrea is one of our dedicated planning facilitators, passionate about helping individuals and families navigate their planning journey with guidance, support, and a personalized approach. As a mom of two and caregiver for her grandfather with dementia, she has firsthand experience balancing family life, caregiving, and hectic schedules.
Fun fact: Andrea’s current hobbies are pottery and power-lifting. Her ability to juggle multiple priorities across family and caregiving responsibilities is a skill she brings to every planning session.

Who is this program for?
This program is designed for adults (18+) with an intellectual or developmental disability and is reimbursable using Passport Funding. If you do not have Passport Funding or are not an Ontario resident, you can still participate.
What will this program accomplish?
This program will support you in developing independence, building life skills, and strengthening community connections. Together, we will define your personal goals, create a clear plan for your next steps, and explore what independence looks like for you. Whether you're transitioning into employment, independent living, or making important life decisions, we’ll guide you and your loved ones through the process to help you gain confidence and independence.
How long is the program?
The program runs for one year, giving you full access to LifeCourseOnline for 12 months, along with 12 planning sessions (one per month).
What if I don’t have passport funding or am not a resident of Ontario?

Can family or caregivers participate? Can you work with other service providers or support teams involved?
Absolutely! We encourage family members, caregivers, and other support professionals to be part of the process. Their involvement provides valuable insights and helps create a plan that considers all the important people in your life.
What is the cancellation or reschedule policy?
Please refer to our Terms of Service for more information on our cancellation and reschedule policy.
Is the program offered in person?
This program is offered virtually via Zoom or Teams. This allows for greater flexibility, accessibility, and convenience.
What if my Passport Funding claim is denied?
Our program has been approved for Passport Funding through PassportONE. If your claim is denied, we will work with you and contact your Passport Agency directly to understand the reason for the denial and help resolve any issues. For more details on our policies and next steps, please refer to our Terms of Service.
What is your availability, and how flexible are you with scheduling sessions?
Our planners are available to help from Mondays to Friday, 9-5 EST. If you cannot meet during these hours, please contact us.
How much does the program cost?
The program is $2200.00 plus tax. It is reimbursable through Ontario's Passport program and includes twelve 1:1 sessions with an LCO planning facilitator.
